What to expect
Stations, timings and how you’re scored
You apply at ST1 for run-through training, and in 2026 the interview carried 70 per cent of the overall selection score. In 2026 NHS England recorded about 23.6 applications for each post.
The Clinical Station
15 minutes
Clinical skills
5 minutes of reading firstYou read a neurosurgical emergency first, such as a deteriorating head injury or suspected cauda equina, pitched at the level of a safe foundation doctor.
The panel wants you to recognise the emergency, resuscitate, arrange imaging and escalate early. In 2026 it was the first station used to break ties.
19 clinical scenarios included.
The Management Station
15 minutes
Organisation and planning
5 minutes of reading firstAn on-call dilemma beyond the clinical facts: competing demands, consent and capacity, a struggling colleague, or no bed.
You work it through to a safe, justified plan, using the team and escalating honestly.
10 management scenarios included.
The Telephone Referrals Station
15 minutes
Communication
5 minutes of reading firstA simulated phone call with no visual cues: you take a referral in or escalate a patient out.
Everything has to happen on the call: gather the story, rank the urgency, give safe interim advice and agree who does what.
11 telephone referral scenarios included.
The Motivation and Understanding of the Specialty Station
15 minutes
Commitment to specialty
5 minutes of reading firstWhy neurosurgery, how you have tested that choice, and what the run-through pathway and the job demand.
Back each reason with something you have actually seen or done, so it comes across as an informed choice.
22 motivation scenarios included.
How the final rank is scored
2026 split. For 2027 a self-assessment with uploaded evidence replaces the portfolio matrix, and you still sit the MSRA. In 2026 you needed 65 per cent of interview marks at ST1, or 74 per cent at ST2.
